{"id":93,"date":"2013-03-12T11:33:58","date_gmt":"2013-03-12T15:33:58","guid":{"rendered":"https:\/\/nolaro24.com\/wordpress\/?p=93"},"modified":"2013-03-12T11:33:58","modified_gmt":"2013-03-12T15:33:59","slug":"qa-with-dr-louis-j-decaro-dpm","status":"publish","type":"post","link":"https:\/\/nolaro24.com\/wordpress\/qa-with-dr-louis-j-decaro-dpm\/","title":{"rendered":"Q&#038;A with Dr. Louis J DeCaro DPM"},"content":{"rendered":"<p><span style=\"color: #ff0000;\"><strong>Q:<\/strong>\u00a0<\/span>What is your feeling\u00a0about the effectiveness of\u00a0splinting to manage foot\u00a0deformities once children\u00a0become early ambulators?<\/p>\n<p><span style=\"color: #0000ff;\"><strong>A:\u00a0\u00a0<\/strong><\/span>With reference to most\u00a0early foot deformities, I am a big believer\u00a0in controlling the calcaneus\u00a0because, until the age of six, the\u00a0calcaneus plays a huge role in creating\u00a0forces on the rest of the foot.\u00a0Therefore, in a patient with \u00a0juvenile\u00a0HAV\/hammertoe, controlling the\u00a0heel in varus in a very early ambulator\u00a0will prove effective at slowing\u00a0down whatever deformity may be\u00a0there or waiting to worsen. In other\u00a0words, I splint with a deep heel cup.\u00a0I don\u2019t think that taping joints to\u00a0change angulations is particularly\u00a0successful. I would, however, never\u00a0discourage parents from keeping\u00a0certain joints splinted in improved\u00a0positions.\u00a0As long as the biomechanics are\u00a0not being compromised through\u00a0splintage, allowing the ligaments\u00a0and joint capsule to set t le in a\u00a0more corrected position during maturity\u00a0can only help the situation.<\/p>\n<p>In cases of metatarsus adductus, in particular, I am a\u00a0frequent user of the Wheaton brace. I find that splinting\u00a0the foot nightly in a more corrective position helps\u00a0to reshape and\/or hold the midtarsal ligamentous\u00a0structure.<\/p>\n<p><strong><em>Note:\u00a0<\/em><\/strong><\/p>\n<p><em>Louis DeCaro, DPM specializes\u00a0in pediatrics with a special interest in\u00a0sports medicine and biomechanics\u00a0for both adults and pediatrics.
